Financial Arrangements for Therapy

Worry about cost should never be the only thing that keeps someone from getting help. While fees vary from one therapist to another, there are usually more options than people realize. This page gives a general overview of how payment for therapy commonly works, so you can ask informed questions when you begin.

Insurance and Managed Care

Many people use insurance to help cover the cost of therapy. As described in more detail on our using insurance page, insurers increasingly rely on managed care: they pay for part or all of a set of pre-authorized sessions with in-network therapists, often with a limit on the number of visits and a review process that keeps the work focused on specific problems. This can be a good fit for shorter-term, well-defined concerns, though it may limit your choice of therapist and involve sharing some information with the insurer.

Paying Out of Pocket

Some people prefer to pay for therapy themselves — whether to keep the work fully private, to see a therapist who is not in their plan's network, or to continue longer than insurance would allow. Paying out of pocket gives you the most freedom to choose your therapist and to shape the length and direction of the work around your own needs rather than a plan's rules.

Sliding Scale and Flexible Options

Cost need not be a barrier. Many therapists offer alternate payment arrangements to make care more accessible, such as:

  • Sliding-scale fees — a reduced rate based on your income or financial situation
  • Deferred or flexible payment plans — arrangements that spread the cost over time

It is always appropriate to ask a prospective therapist about fees and about any flexibility they can offer; most are accustomed to the conversation and glad to have it openly.
